What to expect

Bring your insurance card and a photo ID to your first visit. Plan to arrive 15 minutes early to complete registration and consent paperwork. If you would prefer to complete the paperwork ahead of time, you can print and complete the forms on this page and bring them with you to your appointment. (The Medicare Opt-Out Agreement is only for patients who have Medicare; it is required for all patients who have Medicare as their primary or secondary insurance .)*

For your first appointment, plan for about an hour and a half which includes check-in, time with Lisa, and check out.   When you arrive, our office staff will register you and then check your weight, height and blood pressure.  Next you'll meet with Lisa. During your time with her you will discuss your health history and any health concerns you would like to work on. You will have a physical exam, including a breast and pelvic exam. You may or may not need a pap smear, but all first appointments with Lisa will include a complete physical exam unless you are referred for a consultation.  Lisa will analyze your symptoms and history to determine appropriate bloodwork and/or saliva testing.

  • Bloodwork is done at the lab of your choice and can be submitted to your insurance. We provide the lab order form, which you take with you to the lab to have your blood drawn. The results are sent to our office and Lisa will discuss the results with you and make her recommendations at your follow-up appointment.
  • Saliva testing is done by ZRT Laboratory and is not billable to your insurance. You purchase the test kit in our office, collect the saliva samples at home, then mail the kit to the lab in Oregon. The results are sent to our office and Lisa will discuss the results with you and make her recommendations at your follow-up appointment.

Self-pay patients - 20% discount

If you do not have insurance or we do not accept your insurance company, you may be seen as a self-pay patient. This means you are responsible for the cost of your visit at the time of service. We do not offer payment plans, but we do offer a self-pay discount of 20% if you pay with cash, debit or credit. Checks are accepted, but the discount will not apply.

If you have insurance, we will be happy to provide a copy of your encounter form and proof of payment for you to submit to your insurance. Depending on your insurance plan, you might receive partial reimbursements or the amount you pay might be applied to your out-of-network deductible.

*If you have Medicare, you will not be able to submit your visit for reimbursement. You will need to read and sign our Medicare Opt-Out agreement before you can be seen. The agreement applies only to Lisa J. Roberts's practice, Femme Care Inc. and does not affect your benefits with other providers.

If you would like an estimate for the cost of your visit, please call our office at 540.414.8585.

Insurance

We accept the following insurances

  • Aetna
  • Anthem/BCBS (excluding Healthkeepers)
  • Coventry
  • Optima
  • Tricare

Because individual coverage varies, we recommend you call your insurance before your visit to better understand your possible out-of-pocket expenses. Although Lisa will do a complete physical, the visit will only be billed as a wellness visit if it has been a year or more since your last gynecological wellness visit. If you are due for your annual gynecological wellness visit, we will not collect a copay. For all other follow-ups and office visits, we will collect a specialist copay if listed on your insurance card.
